On behalf of our client in public health, we are seeking a dedicated and compassionate Care Team Social Worker to join their care team within a hospital setting. This is an excellent opportunity for an experienced Social Worker who is passionate about delivering high-quality, patient-centred care and supporting individuals and families throughout their healthcare journey.
Key Responsibilities
- Undertake comprehensive psychosocial assessments of patients and families.
- Provide support, advocacy, and crisis intervention where appropriate.
- Collaborate closely with medical, nursing, and allied health professionals as part of a multidisciplinary team.
- Facilitate discharge planning and coordinate access to community and support services.
- Promote patient wellbeing, safeguarding, and continuity of care.
- Maintain accurate and confidential records in line with professional and organisational standards.
- Participate in case conferences and contribute to service improvement initiatives.
Requirements
- A recognised qualification in Social Work.
- Registration with CORU, or eligibility for registration.
- Previous experience working in a hospital, healthcare, or community setting is desirable.
- Excellent communication, interpersonal, and organisational skills.
- Ability to manage a diverse caseload and work effectively within a multidisciplinary team.
- Knowledge of the Irish healthcare system and available support services.
