Social Worker

On behalf of our client in public health, we are seeking a dedicated and compassionate Care Team Social Worker to join their care team within a hospital setting. This is an excellent opportunity for an experienced Social Worker who is passionate about delivering high-quality, patient-centred care and supporting individuals and families throughout their healthcare journey.

Key Responsibilities

• Undertake comprehensive psychosocial assessments of patients and families.
• Provide support, advocacy, and crisis intervention where appropriate.
• Collaborate closely with medical, nursing, and allied health professionals as part of a multidisciplinary team.
• Facilitate discharge planning and coordinate access to community and support services.
• Promote patient wellbeing, safeguarding, and continuity of care.
• Maintain accurate and confidential records in line with professional and organisational standards.
• Participate in case conferences and contribute to service improvement initiatives.

Requirements

• A recognised qualification in Social Work.
• Registration with CORU, or eligibility for registration.
• Previous experience working in a hospital, healthcare, or community setting is desirable.
• Excellent communication, interpersonal, and organisational skills.
• Ability to manage a diverse caseload and work effectively within a multidisciplinary team.
• Knowledge of the Irish healthcare system and available support services.
